Actively Promoting Change provide a range of services to support individuals to live as independently as possible. Our services promote safety, independence, physical and mental wellbeing and individualisation.
Services we offer include:
- 16+ supported accommodation for young people in care (group living and solo placements available)
- 16+ floating support services (with or without accommodation)
- 16+ short term crisis provision
- 18+ supported accommodation for adults with Autism, Learning Disabilities, Mental Health and other needs
- 18+ outreach care for adults with Autism, Learning Disabilities, Mental Health and other needs
- 18+ respite care
- Other outreach services for children and young people
