Providing high quality care to YOUNG PEOPLE and vulnerable adults

ABOUT US

Our directors and management team have a fantastic depth of knowledge in different areas of support which complements our work. Our backgrounds include, but are not limited to, working with victims of criminal and sexual exploitation, young people and adults with Autism, ADHD and ADD, young people and adults with SEBD and SEMH, young people and adults with alcohol and/or drug addictions, working with homeless individuals and working with victims of domestic abuse.

SUPPORTED LIVING

Our supported living services are decorated, furnished and maintained to a high standard. Weekly Health and Safety checks and regular Management Spot Checks ensure our properties remain safe and welcoming. Personal spaces can be decorated to each individual taste to encourage a sense of inclusion and belonging.

All supported individuals work with their staff teams to create a bespoke support plan to ensure the support provided is targeted, relevant and appropriate to each person.

Our aim is to create a safe and nurturing environment, promoting inclusion, use of own initiative and positive risk taking to achieve the overall aim of improving the confidence and independent living skills of each person.

CLIENT CARE

We support individuals to access employment, volunteering, education, or training opportunities. We also support in registering with local health providers and other individual support agencies such as drug & alcohol services, sexual health services, advocacy services and counselling services. When required, we work with individuals to register with local housing organisations and can provide support to access any available benefits such as Universal Credit.

TRAINING EXCELLENCE

As a company, Actively Promoting Change make full use of the latest modern technology, allowing quick and responsive actions to meet any changing needs. Staff can access care plans, risk assessments, communicate with senior staff and log shift records/handover information all in one place.

All of our supported individuals have full access to any written records which we have completed relating to them.

All of our staff receive ongoing training and guidance from a strong management team to ensure they are up to date with all legislative requirements and to ensure they have the skills needed to meet the individualised needs of each person receiving support from us.
